  1. Open the form using the above link.
  2. Fill in your First name, Last name, and you can also fill the middle name if you have one.

  3. Scroll down to the bottom, and select the reason for changing your name. Try to provide an appropriate reason.

  4. Click on the Choose files button and upload a Government-issued identity proof. Facebook accepts different kinds of identity proofs like (you can get the full list of accepted documents here):
    • Birth Certificate
    • Personal Insurance or Vehicle Insurance card

    • Name-change certificate issued by your country’s Government.

Take a clear picture of the document you’re submitting. Facebook could potentially decline the request if the document or its content isn’t clearly visible.

Things to Remember and Do after Submitting a Name Change Request

After you submit the name change request to Facebook, it’ll take a few days for Facebook to verify your request. Sometimes it may take 4, 5, or maybe even a whole week to get a reply from Facebook. There’s no fixed date that Facebook will reply to you, so we aren’t sure about that either.
